CTSC pays a major portion of your dental insurance premium and your portion is a pre-tax deduction. You have the opportunity to elect coverage that meets your individual and family needs. Coverage options include:

• Employee Only
• Employee and Child(ren)
• Employee and Spouse
• Employee and Family

The dental insurance plan allows you to select the dentist of your choice. The following services are covered at the listed percentages:

The maximum benefit is $1500 per calendar year for each covered person. The calendar year deductible is $50 per person with a maximum of $150 per family and the deductible is only applied to restorative care and major care.
